Why Does a Classic Tummy Tuck Stop at Flat?

A traditional tummy tuck is a front-of-the-body operation. It removes loose skin below the belly button, repairs muscles stretched by pregnancy or weight loss and trims a little fat from the waist. It does those jobs well, and for plenty of patients, it's the right call.

The catch is everything happening at the sides. When the flanks keep their fullness and the hips keep their shape, your waist-to-hip ratio stays about the same. Surgeons writing in Plastic and Reconstructive Surgery Global Open put a word to it, noting that tummy tucks built on muscle plication and side liposuction often left patients with "boxy" profiles. Their review of 1,125 patients found that adding hip fat grafting to abdominoplasty could be done safely and help achieve a more harmonious hip-to-waist ratio. Add: Hip Fat Transfer, Where Your Curves Come From

This is the part patients get most excited about. The fat removed from your sides doesn't go to waste. It's purified and placed into your hips, filling hip dips and building soft shape below your newly snatched waist.

Hip dips aren't a fitness problem, by the way. They come from anatomy. In an area called the trochanteric depression, the skin sits closer to the hip bone, so no amount of squats will fill it in. Your own fat can, and it's the most natural material available.

Tighten: The Internal Corset

Every Smart Tuck includes full muscle repair from the top of the abdomen to the pubic area, and Dr. Earle designs it to work as an internal corset. "We're shrinking the waist from the outside and the inside," he explains.

Where Does Every CC Go? The Smart Tuck™ and Florida's 1-Liter Rule

Here's something a lot of flashy Miami ads leave out. Under Florida Administrative Code Rule 64B8-9.009, liposuction combined with a tummy tuck can't exceed 1,000 cubic centimeters (1 liter) of fat removed. Liposuction on its own can remove up to 4,000 cc in the office setting.

A liter sounds like plenty until you're sculpting an entire torso, so the PURE team plans every cc before surgery begins. The waist, flanks and axillary area get priority. The central abdomen gets etching instead of heavy suction to protect the skin while it heals. Fat removed with the excised skin doesn't count toward the limit, because it's cut away rather than suctioned.

The hips can typically receive about 500 cc per side, which Dr. Earle calls "more than enough to create that nice, beautiful curve without going too crazy or exaggerated."
