Ultra-Snatched Waist: Who Is a Candidate for Rib Remodeling?

You know the typical formula for getting a slim, sculpted figure:

Eat healthy.
Drink water.
Track macros.
Exercise regularly.
Try shapewear.

So why does it feel like your waist hasn't gotten the memo?

At PURE Plastic Surgery, we see this frequently. Patients arrive having followed a strict health regimen for months, often years, but they don't see results where they want them. Maybe they've tried losing weight with GLP-1 medications or removing stubborn fat with liposuction, yet their waist retains a boxy shape instead of the curves they dream of.

If any of this sounds familiar, your ribcage could be the missing piece of the puzzle. Wide or flared lower ribs set a limit on how much you can whittle your waistline, and no amount of healthy living can change their angle. But for the right candidate, rib remodeling can.

What Is Rib Remodeling?

Rib remodeling is a minimally invasive surgical technique that reshapes the floating ribs (ribs 10, 11 and 12) to narrow the waist. This is an exciting option for many patients because:

  • No ribs are removed during the procedure
  • It is less invasive, painful and risky than rib removal surgery
  • The procedure is done through 18-gauge needle punctures rather than linear incisions
  • The resulting marks typically fade to the point of being invisible, making it a virtually scarless technique
  • Results are permanent once the bone heals in its new position

At PURE Plastic Surgery, rib remodeling is performed by double-board-certified plastic surgeon Dr. S. Alexander Earle. Dr. Earle trained in this technique with Dr. Alfredo Hoyos' group in Colombia, the team that originated it. He was one of the first plastic surgeons to bring it to the United States.

What Makes Someone a Good Candidate for Rib Remodeling?

You may want to consider rib remodeling if you have wide or flared lower ribs that give your waist a straight shape, even at a lean weight. Most patients are women who desire a more defined, hourglass figure. The procedure is also part of body feminization for many transgender women.

A consultation with Dr. Earle is the first step. During this initial appointment, he will evaluate the following factors to determine whether you might be a good candidate for the procedure:

‍

Factor PURE’s Guideline Why It Matters
Body Composition Rib shape is the underlying issue If the underlying issue is muscle weakness or excess fat, a different procedure is appropriate
BMI 25 or below Dr. Earle needs to be able to feel the ribs through the overlying tissue to perform the procedure
Age Under 40 is ideal; over 45 is generally not recommended Bones must be healthy to heal properly, and the risk of osteoporosis increases with age
Rib Anatomy CT scan with 3D rib reconstruction required before surgery The scan allows Dr. Earle to visualize the ribs so he can see your unique anatomy and plan the procedure effectively
Recovery Commitment Corset must be worn nearly 24/7 for three months post-surgery Wearing a corset during recovery is non-negotiable; it holds the ribs in their new position while they heal
Weight Stable, not actively losing Reaching a stable, healthy weight before the procedure helps ensure the predictability and stability of the results
Family Planning Ideally complete Pregnancy is possible post-surgery, but it may push the lower ribs outward again and affect the results

Why We Stick to These Guidelines at PURE Plastic Surgery

Safety is the foundation we've built our reputation on. We call it The PURE Way: the combination of Smart Aesthetics™, Surgical Intelligence™ and Modern Recovery that helps us plan and personalize your procedure, perform the surgery with the latest techniques and sound clinical judgment, and guide you through a calm, comfortable and healthy recovery.

Being realistic about your candidacy for a procedure is one of the most critical steps of this process. Here's why we've established these guidelines for patients considering rib remodeling in Miami:

Body Composition

Everyone is built differently. Some patients have a larger, boxier waist shape because of the structure of their ribs. Other patients have a small ribcage but a similarly straight shape because they have excess fat in their midsection or because their abdominal muscles separated during pregnancy. There may be a combination of factors at play.

There are three ways to bring in the waist:

  • Rib remodeling to sculpt a narrower lower ribcage
  • Liposuction to remove fat from the waist, flanks and abdomen
  • Tummy tuck to tighten the muscles (muscle plication)

Your surgeon's job is to identify the underlying reason for your concerns and match the correct procedure to the cause. You will not get the best result if this is not done thoughtfully.

BMI

Rib remodeling is for lower BMI patients — typically a BMI of 25 or less. "If there's too much fat between the skin and the ribs and I can't feel them, I can't do the procedure," Dr. Earle explains.

For patients above a BMI of 25, liposculpture alone often achieves a dramatic change. Rib remodeling may be unnecessary to achieve the patient's goals.

For patients who are already slim or athletic and have a straight waist, there is little to no fat that can be removed. Instead, rib remodeling can be a powerful way to shrink the waist and create a curvier, yet natural-looking, shape.

Age

Rib remodeling has a stricter age limit than many cosmetic surgeries because it deals with bone. The bone must be healthy enough to heal fully after the procedure.

Bones naturally become more brittle with age, especially for women after menopause. Declining estrogen levels accelerate bone loss and increase the risk of osteoporosis. Many people aren't aware they have the condition until they break a bone.

For this reason, rib remodeling is typically performed on patients who are 40 or younger. "If you're between 40 and 45, we can consider it," Dr. Earle says, "but we will do an additional test called a bone density scan to make sure your bones are in good shape."

Rib Anatomy

All patients are required to undergo a CT scan with 3D rib reconstruction before rib remodeling surgery. As Dr. Earle explains:

"Not everybody has the same anatomy. Some people have a very short 12th rib. Some don't have a 12th rib at all. The 10th rib only floats in about 50% of the population. For all these reasons, I need a CT scan to properly plan your surgery."

An MRI (Magnetic Resonance Imaging) scan cannot substitute for the CT scan. MRI is primarily used to visualize soft tissues, while CT scans are better for evaluating hard bone structures. The good news: CT scans are usually faster and less expensive than MRIs.

Recovery Commitment

After the procedure, the ribs naturally want to spring back to their original position. You must wear a corset to hold them in place while they heal, and it needs to stay on day and night for three months.

The corset looks similar to a waist trainer, with multiple clips so you can adjust the size as you heal. You will gradually bring the clips inward, and depending on how you progress, you may decrease the size of the corset. Some patients switch to a corset with lacing strings in the back after six weeks if they want a more extreme hourglass shape.

Dr. Earle is clear that this is non-negotiable: "If you're not willing or unable to commit to the corset 23 hours a day for three months, don't do the procedure."

Weight & Family Planning

The reason for these guidelines is the same: the less your body changes after surgery, the more likely you are to retain your results.

If you are actively losing weight, or if you gain weight in the future, your waist is a moving target. Maintaining a stable weight before and after the procedure helps Dr. Earle plan around the body you'll be living in and safeguards your newly sculpted shape.

Pregnancy is not forbidden after rib remodeling. Once your ribs have healed (usually after about three months), it's safe to get pregnant. However, you must consider that a growing baby often widens the mother's ribcage. If more children are in your future, you may want to wait to have surgery until your family is complete so you can enjoy your results for as long as possible.

Not a Candidate for Rib Remodeling? Here Are Other Options to Consider

Hearing "it's not for you" or "not right now" can be disappointing, but our surgeons will not make a recommendation that is not in your best interest. We want you to love your results, and we want you to be safe. Sometimes that means suggesting alternative paths forward.

If Dr. Earle determines that rib remodeling is not the right fit for you, you may discuss other options such as:

  • BMI over 25: Lipo 360 to reduce fat around the full waistline; rib remodeling can be considered in the future if BMI decreases
  • Separated abdominal muscles: tummy tuck for muscle repair; may be combined with liposuction for waist sculpting
  • Over 45: Total Definer Package with HD Lipo for comprehensive body sculpting

Rib Remodeling Candidacy FAQs

Can I combine rib remodeling with other procedures?

Yes, you can combine rib remodeling with nearly any other procedure we offer at PURE, including tummy tuck and breast surgeries. The most common procedures we combine it with are Lipo 360 and BBL. Dr. Earle calls this combination the "PURE BBL 2.0" because it takes body contouring to a level none of the procedures can achieve individually.

Can I get rib remodeling if I'm already thin?

Yes. Rib remodeling can narrow a waist that liposuction can't change because there isn't enough fat to remove. Lower-BMI patients are usually the best candidates for rib remodeling, as they do not have fat tissue covering the ribs that prevents Dr. Earle from performing the procedure.

Can I get rib remodeling if I'm over 40?

Possibly. If you are between 40 and 45, you will need to have a bone density scan to ensure your bones are healthy enough to heal well. Rib remodeling is usually not recommended after 45 because bone density tends to decline around menopause.

Can I get rib remodeling if I lose weight?

If your BMI is higher than 25, you may become a candidate for rib remodeling if you lose weight. Some patients start with liposuction and schedule rib modeling at a later date. Others may wish to consider a medical weight loss program to reach an appropriate weight before surgery.

Can I find out if I'm a candidate for rib remodeling without traveling to Miami?

Absolutely. PURE offers virtual consultations for patients who live outside of Miami. If you choose to travel to Miami for surgery, our coordinators will help you book the procedure and make travel arrangements. Visit our Out-of-Town Patients page to learn more.
