Brazilian
Butt Lift (BBL)

Enhance Your Buttocks
Outside of Brazil, Miami is the BBL capital of the world. With more BBL procedures performed here than nearly anywhere else, it’s where techniques, safety and surgical standards have been pushed further than in any other market, with PURE Plastic Surgery leading the way.
At PURE Plastic Surgery, BBLs are performed by our double-board-certified plastic surgeons, Dr. Earle, Dr. Vidal, and Dr. Wegerif, using ultrasound-guided techniques developed and refined in-house. Dr. Earle himself has performed more than 2,000 ultrasound-guided BBLs and pioneered a safety standard the state of Florida eventually made law.
If you're researching a BBL in Miami, you've found the right place to start.
What Is a Brazilian Butt Lift (BBL)?
A Brazilian butt lift is a butt augmentation procedure that uses your own fat, harvested with liposuction from the abdomen, flanks or thighs, and transferred to the buttocks for added volume and improved shape. Because it uses fat grafting rather than implants, it can enhance both the buttocks and the donor areas where fat is removed.

What Concerns Can a BBL Address?
A BBL can improve buttock volume, projection and contour when you want fuller shape and smoother proportions. It is also commonly used to enhance your silhouette by:
- Contouring the waist and flanks with liposuction for a more defined shape
- Reducing fat in the back, abdomen or thighs
- Creating a smoother transition between the waist, hips and buttocks
Together, these changes create a more balanced, proportionate silhouette rather than simply adding volume to the buttocks. Your surgical plan is personalized to your needs and goals, allowing you to target the areas that will have the greatest impact on your overall shape.

Why Do Miami Patients Choose PURE for BBL Surgery?
"I would never do one without ultrasound, and I think that really is the way to go. All the people that are really in the know — those at the cutting edge with the technology who really want to keep their patients safe — they're going to use ultrasound for their BBLs." — Dr. Earle

Ultrasound-Guided Brazilian Butt Lift (BBL)
Every BBL performed at PURE Plastic Surgery uses real-time ultrasound imaging during fat transfer for enhanced patient safety. Dr. Earle has been using ultrasound guidance since 2019, years before it was legally required in Florida, refining his technique. His standards have been adopted by our entire team, and they’re the reason PURE's outcomes are what they are.

South Beach Butt Lift (SOBEBL)
The South Beach Butt Lift (SoBeBL) is Dr. Earle’s signature procedure that pairs liposuction with fat transfer to create a natural-looking butt shape with small, discreet incisions. The goal is an enhanced silhouette that looks balanced from every angle, including a smooth transition from waist to hips. It’s this level of intentional, proportion-driven planning that sets PURE apart.

The Earle Curve
The Earle Curve is a term that patients use to describe Dr. Earle’s signature result: a smooth, balanced hourglass figure from the front, and an upside-down heart from the back. In practice, this means your BBL is planned with torso and back contouring. Rather than focusing just on volume, the Earle Curve produces a curvy silhouette with a defined waist-to-hop transition, prioritizing natural, proportional results that work with your foundation rather than against it.
Why does PURE Plastic Surgery use ultrasound guidance for BBL procedures?
PURE Plastic Surgery uses ultrasound imaging during fat transfer to support precise placement of fat. Ultrasound guidance helps the surgeon visualize anatomy during injection, which is intended to reduce the risk of fat being placed in unsafe areas.

What Butt Augmentation Options Are Available at PURE Plastic Surgery?
At PURE Plastic Surgery, butt enhancement options include ultrasound-guided fat transfer (BBL techniques), Dr. Earle’s signature South Beach Butt Lift (SoBeBL), and surgical buttock lift-style procedures intended for loose skin rather than volume. Your consultation determines which approach matches your anatomy, goals, and tissue characteristics.
Can a BBL Be Combined With Other Procedures?
Absolutely. At PURE, BBLs are often combined with additional procedures to refine and enhance your results. Some of our most popular combinations include:
Rib Remodeling
Rib remodeling has become one of our most requested add-ons to the Brazilian butt lift. This advanced procedure repositions the lower ribcage to create a narrower waistline, amplifying the curves achieved through fat transfer. The result is a dramatically enhanced hourglass figure with a smooth transition from waist to hips. PURE’s own Dr. Earle is one of the few plastic surgeons in the United States performing this technique.
Energy-Based Skin Tightening
If you have loose skin around the midsection, adding a skin-tightening treatment to your BBL can also enhance your final contours. Energy-based devices like BodyTite (radiofrequency) or Renuvion J Plasma (helium plasma) stimulate collagen production and help the skin adhere more smoothly to your new shape.
The V-Lift
The V-Lift is a new procedure at PURE Plastic Surgery that combines a surgical buttock lift with liposuction of the entire back and fat transfer to the hips and buttocks. While a BBL focuses on adding volume, the V-Lift is more like a modification of the lower body lift, providing comprehensive reshaping and addressing the irregularities that can occur in the hips and buttocks due to skin laxity, leaving you with beautifully contoured results.
Can I Combine a BBL with a Tummy Tuck?
Pairing a BBL with a tummy tuck is one combination procedure we won’t do, and the reason is simple: because your safety comes first, always.
Combining a BBL with a tummy tuck raises the risk of complications to a level our surgeons aren’t willing to accept. The recovery process also becomes contradictory. For a BBL, you need to be off your buttocks, but for a tummy tuck, you need to be off your stomach. Even with pillows and other recovery aids, more pressure is going to be put on the buttocks, compromising your final results.
Lastly, the amount of fat that can be harvested with excisional procedures like tummy tucks is limited to 1,000 cc. That’s enough for strategic contouring with the Smart Tuck, but not enough for a BBL. You’d be accepting higher risks for lesser results.
Am I a good candidate for a Brazilian Butt Lift?
A good candidate for a BBL has enough donor fat to safely harvest and transfer, along with goals that align with their natural proportions. Candidacy also depends on your overall health, skin quality and weight stability.
You may be a strong candidate if you:
- Want more buttock volume, projection or a more defined shape
- Have enough donor fat in areas like the abdomen, flanks, back or thighs
- Have a BMI between 21 and 30
- Are at a stable weight and have maintained it for several months
- Are in good overall health and do not smoke
- Have realistic expectations based on your body type and available fat
Patients with a BMI below 21 may not have sufficient donor fat for a traditional BBL. In these cases, the South Beach Butt Lift (SoBeBL) with VASER technology can help maximize fat extraction in lower-BMI patients.

What Should I Expect at My BBL Consultation?
Your consultation is where your surgeon confirms candidacy, reviews your medical history, evaluates donor fat areas, and discusses realistic outcomes based on your proportions. You will also review surgical planning, recovery expectations, and any recommended complementary procedures or skin-tightening options if needed.
Bring:
- A list of your medications and supplements
- Your medical history and prior surgery information
- Photos of your goal shape (optional, helpful for communication)
- Your top questions about safety, recovery, and results
You will leave with a clearer plan for donor areas, anticipated contour changes, recovery logistics, and next steps for scheduling. To schedule your consultation today, call our Miami office at 305-230-4326 or get started with our virtual booking tool.
What Happens During BBL Surgery?
BBL surgery at PURE is performed under general anesthesia at our Joint Commission-accredited surgical suite in Miami, which is the highest level of accreditation for office-based surgery. Your BBL is carefully planned with your goals and natural proportions in mind to reshape your overall silhouette, not just add volume.
The procedure typically follows these steps:
- Liposuction to harvest fat: Fat is removed from donor areas such as the abdomen, flanks, back or thighs. VASER ultrasound technology may be used to maximize fat extraction while supporting smoother contouring and skin retraction.
- Fat processing: The harvested fat is purified so only healthy fat cells are used for transfer.
- Ultrasound-guided fat transfer: Fat is precisely placed into the subcutaneous space of the buttocks using real-time ultrasound imaging, allowing for controlled, layer-by-layer shaping of projection and contour.
- Closure and recovery: Small incisions are closed, and you are monitored in recovery as anesthesia wears off before being discharged with detailed aftercare instructions.

What Is the BBL Recovery Like?
BBL recovery includes swelling, bruising, and soreness, plus activity and positioning restrictions to protect transferred fat. A key recovery principle is minimizing pressure on the buttocks, including using a support pillow when sitting and choosing safe sleep positions.

What Does the First Week After a BBL Involve?
The first week commonly includes the most noticeable swelling and soreness, limited activity, and a focus on walking briefly for circulation while avoiding strain. You will follow instructions on compression garments, incision care, and positioning to avoid direct pressure on the buttocks.
What Is a Realistic Week-By-Week BBL Recovery Timeline?
Recovery varies by patient, but most follow a similar pattern as swelling resolves and fat settles. Here is what a typical BBL recovery timeline looks like after surgery:
- Days 1–7: Peak swelling, soreness and fatigue. Focus on short walks, wearing compression garments and avoiding direct pressure on the buttocks.
- Weeks 2–3: Bruising and swelling begin to improve. Continue pressure avoidance and follow garment guidelines as directed.
- Weeks 4–6: Ongoing healing with gradual improvement in comfort. Maintain positioning awareness and ease back into activity as cleared.
- Months 3–4: Results appear more settled as swelling decreases. This is when fat retention and overall shape become clearer.

What Aftercare Instructions Help Protect BBL Results?
Protecting transferred fat relies heavily on positioning and following post-op instructions closely. Many patients are instructed to avoid direct sitting on the buttocks for a period of time, use a support pillow when sitting is necessary, sleep on the stomach or sides, and follow compression garment guidance for liposuctioned areas.
Nutrition, hydration, and appropriate activity progression also matter because recovery affects swelling, comfort, and the stabilization of results.

What Side Effects and Risks Should I Understand Before a BBL?
BBL surgery has risks like any procedure involving anesthesia, liposuction, and fat transfer. Your surgeon will review risks and safety protocols during consultation, including how ultrasound guidance supports visualization during fat transfer.
If you develop urgent symptoms after surgery, such as chest pain, shortness of breath, severe swelling in one leg, high fever, uncontrolled bleeding, or worsening pain, seek emergency care.

When Will I See Final BBL Results, and How Long Do They Last?
You will see an immediate change, but final results take time as swelling decreases and fat retention becomes clearer. Long-term results depend on fat survival, weight stability, and natural aging, because weight changes can expand or shrink fat cells over time.
How Much of the Transferred Fat Typically Survives?
Not all transferred fat survives the healing process. BBL planning commonly accounts for partial resorption, and aftercare is designed to support fat survival by minimizing pressure on the buttocks and following recovery instructions closely.
