A good candidate for a BBL has enough donor fat to safely harvest and transfer, along with goals that align with their natural proportions. Candidacy also depends on your overall health, skin quality and weight stability.

You may be a strong candidate if you:

  • Want more buttock volume, projection or a more defined shape
  • Have enough donor fat in areas like the abdomen, flanks, back or thighs
  • Have a BMI between 21 and 30
  • Are at a stable weight and have maintained it for several months
  • Are in good overall health and do not smoke
  • Have realistic expectations based on your body type and available fat

Patients with a BMI below 21 may not have sufficient donor fat for a traditional BBL. In these cases, the South Beach Butt Lift (SoBeBL) with VASER technology can help maximize fat extraction in lower-BMI patients.

What Happens During BBL Surgery?

BBL surgery at PURE is performed under general anesthesia at our Joint Commission-accredited surgical suite in Miami, which is the highest level of accreditation for office-based surgery. Your BBL is carefully planned with your goals and natural proportions in mind to reshape your overall silhouette, not just add volume.

The procedure typically follows these steps:

  • Liposuction to harvest fat: Fat is removed from donor areas such as the abdomen, flanks, back or thighs. VASER ultrasound technology may be used to maximize fat extraction while supporting smoother contouring and skin retraction.
  • Fat processing: The harvested fat is purified so only healthy fat cells are used for transfer.
  • Ultrasound-guided fat transfer: Fat is precisely placed into the subcutaneous space of the buttocks using real-time ultrasound imaging, allowing for controlled, layer-by-layer shaping of projection and contour.
  • Closure and recovery: Small incisions are closed, and you are monitored in recovery as anesthesia wears off before being discharged with detailed aftercare instructions.

When Will I See Final BBL Results, and How Long Do They Last?

You will see an immediate change, but final results take time as swelling decreases and fat retention becomes clearer. Long-term results depend on fat survival, weight stability, and natural aging, because weight changes can expand or shrink fat cells over time.

How Much of the Transferred Fat Typically Survives?

Not all transferred fat survives the healing process. BBL planning commonly accounts for partial resorption, and aftercare is designed to support fat survival by minimizing pressure on the buttocks and following recovery instructions closely.
