Understanding the Recovery Process for a BBL

A butt lift can improve the shape of the buttocks while also slimming other areas of the body through fat removal. The goal is to create smooth transitions between the hips, buttocks, and lower back for a soft, round shape. When done correctly, it can enhance your natural proportions without obvious signs of surgery.

At PURE Plastic Surgery in Miami, FL, we take this procedure further with proprietary techniques that reduce visible scarring, increase safety and improve fat retention. These methods also support a smoother recovery process by minimizing trauma to the surrounding tissue. We’ll walk you through what to expect during recovery after our unique South Beach Butt Lift.

What Is the South Beach Butt Lift (SoBeBL)?

The South Beach Butt Lift, our signature spin on the “skinny BBL,” uses liposuction and fat transfer to enhance your buttocks and hips in a single procedure. We use fat from areas like the abdomen, waist or back, then inject it strategically to create smoother transitions and natural-looking volume. Instead of placing incisions in obvious areas, we use small access points that are hidden in natural creases. That allows you to wear swimsuits or fitted clothing without drawing attention to scars. We design the shape based on your existing structure so the result complements your body from every angle.

Our skinny BBL provides natural results that keep people guessing, and is suitable for lower-BMI patients (<25) who might not have enough fat for a traditional BBL. Depending on the amount of volume desired, a no-liposuction, off-the-shelf fat transfer with AlloClae, may also be recommended for patients with a lower BMI, ideal for filling in small dents or subtly enhancing the hips, buttocks or breasts. During your consultation, our team will help you decide whether a traditional BBL, the skinny SoBeBL or AlloClae is right for you

What Makes the Ultrasound-Guided SoBeBL Special?

During this procedure, we use ultrasound imaging that allows us to see the gluteal anatomy in real time during your surgery. Because of this technology, we can place fat precisely into the safest subcutaneous layers. This technique helps avoid major blood vessels and reduces the risk of complications, including fat embolism. SoBeBL also uses tiny, hidden incisions instead of the multiple access points used in traditional fat transfer. The result is minimal visible scarring, even in revealing clothing, and a more refined, natural contour.

An Overview of the SoBeBL Recovery Timeline

Immediate Post-Op

Right after surgery, you will spend some time in a monitored recovery area as the anesthesia wears off. Expect to feel groggy, sore and swollen, which is normal after fat transfer and liposuction. We will give you detailed instructions before you leave, including how to sit, sleep and care for your incisions. Arrange for someone to drive you home and stay with you for at least 24 hours.

Week 1–2

Swelling and bruising tend to peak during the first week, and soreness can continue during this time. You should continue avoiding direct pressure on the buttocks to protect the newly transferred fat. Wear your compression garment as directed to help reduce swelling and support the healing process. You may feel well enough to move around the house, but avoid lifting, bending or standing for long periods.

Weeks 3–6

By week three, most patients feel more mobile and ready to return to desk work or light activities. It is still important to avoid intense workouts or anything that puts pressure on the treated areas. Swelling may still be present, but you will start to see more shape and definition as your body heals. We will guide you through the next steps and let you know when it is safe to resume regular activities.

Months 2–6

During this stage, the transferred fat stabilizes, and swelling gradually resolves. The volume you see now is closer to your long-term result, although subtle changes can still happen. If you maintain your weight and follow your care instructions, the outcome should remain consistent. You can resume full workouts and more active movement at the six-week mark or once cleared. At this point, the incisions have usually faded, and your new contours continue to look and feel more natural.

Our Aftercare Recommendations

Positioning Your Body After a SoBeBL

Avoid sitting directly on your buttocks for at least six weeks. If you need to sit, use a support pillow that keeps pressure off the treated area and shifts your weight to your thighs. For sleep, lie on your stomach or side and use pillows to support your position and prevent rolling onto your back. Keeping pressure off the fat transfer sites gives the fat cells the best chance to survive.

Wearing Your Compression Garment

Wear your compression garments as instructed, usually day and night, for the first one to two weeks. These garments reduce swelling, support the newly contoured areas and improve how the treatment area settles. We include the initial garment with your procedure, and we will show you how to put it on and take it off correctly. Make sure the garment fits snugly without folding or digging into the skin.

Managing Pain, Swelling and Discomfort

Expect soreness, tightness and some bruising during the first few days. We will provide pain medication to help you stay comfortable and may recommend over-the-counter options as healing progresses. Apply cold packs to reduce swelling, but avoid placing them directly on the buttocks. Gentle walking around the house helps circulation and can ease stiffness.

Keeping the Area Clean

Follow the cleaning instructions we give you to avoid infection and promote healing. Gently wash around your incisions with surgical soap, then pat dry with a clean towel. Do not scrub, soak in baths or apply lotions unless directed. Keep an eye out for redness, unusual drainage or anything that looks or feels different than expected.

Planning Your Activity Levels

Start with short, easy walks each day to improve circulation and reduce the risk of complications. Avoid lifting, bending or stretching during the first couple of weeks. Once you reach the three-week mark, you may begin increasing your activity slowly, but avoid strenuous workouts until we clear you.
