V Lift: How Fat Grafting Enhances Butt Lift Results

Most butt lift conversations start and end with volume, which is fair enough, considering volume is what a BBL delivers beautifully. But if your hips and buttocks have lost some of their definition after major weight loss or pregnancy, volume alone isn’t going to bring back the contours you’re looking for. You need lift alongside volume.
That pairing is the heart of the V Lift, a surgical buttock lift developed at PURE Plastic Surgery in Miami by double board-certified plastic surgeon Dr. Natalia Vidal. This approach combines lifting with fat transfer to create fuller curves. Here's how the two work together and how to tell which approach fits your body.
Why Does the V Lift Include Fat Grafting?
When a surgeon removes excess skin and tightens the tissue underneath, the buttocks sit higher, but they can also look smaller and flatter than before. A lift is subtraction, which can suffice on its own in some cases, but beautiful contours often call for a bit of addition as well.
At PURE, fat grafting is built into the V Lift plan from the start. Dr. Vidal performs liposuction across the entire back and flanks, purifies that fat and transfers it to the hips and buttocks.
This approach:
- Restores fullness that the lift might otherwise reduce, so your shape reads as lifted rather than smaller.
- Smooths contour irregularities that tend to linger when skin has lost some of its elasticity.
- Balances asymmetry, since Dr. Vidal places fat strategically on each side.
- Sculpts your back at the same time, because the donor fat comes from the back and flanks. This sharpens the transition from waist to hips.
Do I Need Fat Grafting, a Lift or Both?
The answer very often comes down to skin elasticity. Volume procedures work best when skin can bounce back and hug new curves. Once skin has stretched past that point, a lift becomes part of the conversation.
Fat Grafting Alone: BBL or SoBeBL
If your skin has good elasticity and your main goal is fuller, rounder buttocks, a Brazilian butt lift (BBL) or Dr. Earle's signature South Beach Butt Lift (SoBeBL) is likely the better fit. Both use your own fat, and every BBL at PURE is performed with real-time ultrasound guidance during fat transfer. PURE generally looks for a BMI between 21 and 30 for a BBL, although the SoBeBL with VASER can help leaner patients make the most of limited donor fat.
Donor fat has limits, though. For patients with very low BMI, gradually gaining weight (and maintaining after the procedure) can help.
A V Lift With Auto Augmentation
Some patients arrive with plenty of loose skin and very little fat to spare. This is common after massive weight loss, when the body has already used up most of its reserves. For these patients, Dr. Vidal uses a technique called auto augmentation.
Instead of removing all the excess tissue from the upper buttock, Dr. Vidal folds some of it inward to build internal volume. It's a reliable and resourceful way to put tissue to work that would otherwise be discarded.
V Lift With Fat Grafting: Why Not Both?
This is where many V Lift patients tend to land. If you have noticeable laxity in the hips and buttocks and enough fat on your back and flanks to transfer, our proprietary V Lift technique gives you both in one procedure: a true lift plus shape-refining fat grafting.
It's especially popular with patients who have already had a tummy tuck or mommy makeover and now want the back of the body to match the front. The V Lift incision can even connect with an existing tummy tuck scar along the hips, creating one continuous, discreet line.
What Is the V Lift With Fat Grafting Experience Like?
Every V Lift starts with a plan, and Dr. Vidal builds that plan with you long before you reach the operating room. Here's a look at what happens from consultation through recovery.
Before Surgery
Your consultation, in person or virtual, is where Dr. Vidal evaluates your skin quality and how much donor fat you have available. She'll also ask “the question”: "Why now?"
"It's important [to ask] because a lot of people have been thinking about a certain area they've wanted to improve for years, but what changed now?" she explains. The answer helps her understand your story along with your anatomy.
Once you've booked, PURE's pre-op team reaches out about 60 days before surgery with your lab orders. Plan to see your primary care physician within this time period, and submit your clearance at least three weeks before your surgery date.
Surgery Day
The V Lift is performed under general anesthesia in PURE's Joint Commission-accredited surgical facility in Miami. It typically takes four to five hours, and Dr. Vidal works in stages:
- Liposuction of the back and flanks: Fat is harvested and purified for transfer. For auto augmentation patients, this step may be modified or skipped.
- The lift: A V-shaped incision is placed in the upper buttocks, following the line where a thong or bikini would sit. Excess skin is removed, and the buttock tissue is lifted and repositioned.
- Fat transfer to the hips and buttocks: Dr. Vidal places the purified fat strategically to refine your shape and balance each side.
- Closure: Incisions are closed in multiple layers, and two drains are placed to manage swelling. You'll wake up in a compression garment.
If your plan includes a 180-degree back lift or liposuction of additional areas, Dr. Vidal keeps the total surgical time to about five hours. It's one of the ways she keeps complication rates low. "Every procedure involves risks, so trying to minimize them is very important," she says.
Recovery
Recovery from a V Lift is more involved than that of a BBL because of the lifting factor. Most patients plan for one to two weeks off work, and the timeline generally looks like this:
- Days 1 to 7: Expect soreness and swelling in the hips and buttocks. You'll wear your compression garment and begin lymphatic massages. Your mandatory in-person post-op visit takes place on day five.
- Weeks 1 to 2: Drains typically come out around this point, and many patients return to light desk work.
- Weeks 2 to 6: Discomfort eases considerably. Avoid direct pressure on your buttocks to protect the healing tissue and transferred fat, and hold off on exercise until Dr. Vidal clears you. Most patients sit normally again around the six-week mark.
- Weeks 6 and beyond: Most swelling has resolved. Your scar continues to mature over the next six to 12 months, fading from pink to a thin, pale line.
One tip for our Miamians and anyone else recovering in a sunny area: keep that incision out of the sun. UV exposure can permanently darken a fresh scar. If you're flying in for surgery, you can typically head home after your day-five visit. PURE partners with AC Hotel Miami Dadeland for out-of-town patients.
What Kind of Results Should I Expect After a V Lift With Fat Grafting?
Picture your hips and buttocks sitting higher with firmer, smoother contours, framed by a more sculpted back. Because the V Lift addresses both laxity and volume, the result tends to look more complete than either approach could deliver on its own. The difference is especially clear after major weight loss.
As with so many procedures, patience is key. Your final shape emerges gradually as swelling resolves and tissues settle into their new position. Some of the transferred fat will also be absorbed naturally in the months after surgery, which is a normal part of the process that Dr. Vidal accounts for when planning the surgery with you.
Finally, while the fat cells that survive the transfer stay put for good, they still behave like every other fat cell in your body, meaning they can expand or shrink with weight changes. The lift itself is structural, so that part of your result holds up well over time.
How Can I Make V Lift Results Last?
Most of the work is done in the operating room. Still, what you do afterward can affect how long you enjoy pristine results. It starts by taking the early recovery window seriously.
Wear your compression garment and avoid pressure on your buttocks during the first several weeks to give the transferred fat its best chance to establish a new blood supply. Keep up with your lymphatic massages, since they help manage swelling as your contours settle.
Beyond the short-term window, weight stability matters more than anything else. Because grafted fat responds to gains and losses just like the rest of your body, a steady weight protects your shape. If you're planning more children, talk with Dr. Vidal about timing.
Why Choose Dr. Natalia Vidal of PURE Plastic Surgery for Your V Lift?
The V Lift is Dr. Vidal's procedure. She developed it at PURE out of her work with two groups of patients she connects with deeply: people who have lost a massive amount of weight and mothers who want to feel like themselves again after pregnancy. As a mom herself, she understands those changes firsthand.
Dr. Vidal’s fat grafting expertise comes with a serious pedigree. PURE founder Dr. S. Alexander Earle, past president of the World Association of Gluteal Surgeons (WAGS) and a pioneer of ultrasound-guided BBLs with more than 2,000 performed, personally trained Dr. Vidal in his BBL and BBL reversal techniques.
Dr. Vidal is certified by both the American Board of Surgery and the American Board of Plastic Surgery. She graduated at the top of her class from Ponce School of Medicine and served as administrative chief resident during her general surgery residency at the University of Puerto Rico. From there, she completed two fellowships. The first was in burn and reconstructive surgery at Albert Einstein College of Medicine in New York City, and the second was in plastic and reconstructive surgery at the University of Miami.
Finally, Dr. Vidal is bilingual, and her YouTube series, Tuesday Tips With Dr. Natalia Vidal, answers patient questions in both English and Spanish. Every surgery takes place in PURE's Joint Commission-accredited facility, which holds the highest level of accreditation available for office-based surgery.
