What Is Motiva Preservé?
Motiva Preservé is a tissue-preservation approach to breast augmentation that keeps your natural anatomy fully intact. That means breast tissue, ligaments, nerves, and muscle are all preserved. The result is a faster recovery, a natural feel, and an implant that works with your body rather than against it.
Fewer than 100 surgeons across the entire United States are trained to perform Motiva Preservé, and PURE is one of a handful of practices in Miami to offer it. Dr. Earle is a Sulàyöm graduate, which means he has visited the Motiva headquarters in Costa Rica to complete specialized, in-person training at the Sulàyöm Global Learning Center.
How Is Motiva Preservé Different Than Traditional Breast Augmentation?
Traditional breast augmentation requires general anesthesia, larger incisions and electrocautery to create the implant pocket, along with insertion typically under or partially under the pectoral muscle (although for the right patient, over-the-muscle placement is possible). Because the chest muscle and tissues are disrupted, recovery tends to be longer, with most patients taking several weeks off from strenuous exercise or activities to heal.
Preservé takes a fundamentally different approach. Here's how the two compare:

What Implant Is Used With Preservé?
Motiva Preservé uses the SmoothSilk® Erognomix implant, which is a super soft implant with a natural look and feel. It’s also highly flexible, allowing it to be inserted through smaller incisions with less disruption to the surrounding tissues, making it ideal for a low-impact breast augmentation technique like Preservé. Ergonomix also incorporates BluSeal® technology and a TrueMonobloc® shell for added confidence and peace of mind.
How Does Motiva Preservé Work?
Motiva Preservé is performed through a small incision and involves no cutting through muscle, no cautery to create the breast pocket, and no destruction of the natural ligaments of the breast. Instead, it uses proprietary tools and a gentler, three-step approach to breast augmentation. Here’s how the Motiva Preservé system works:
- Channel Separator: The Motiva Channel Separator creates a precise pathway through the breast tissue, rotating in a circular motion to reduce trauma.
- Inflatable Balloon: A balloon-like expander creates the implant pocket by gently elongating the tissues without damaging the surrounding structures.
- Insertion Sleeve: The Motiva Insertion Sleeve is a no-touch funnel delivery system that allows the implant to be placed through small incisions.
The no-touch technique is one of the primary reasons this procedure has such a low capsular contracture rate and almost no chance of infection. “Motiva has the lowest capsular contracture rate on the market," Dr. Earle notes. "Less than 1%. And that has to do with their shell, their micro-texture, and of course, being able to place the implant through that no-touch technique."
Am I a Good Candidate for Motiva Preservé?
Preservé is designed for first-time breast augmentation patients who want natural-looking results. The ideal candidate has minimal sagging (grade zero to one ptosis), wants modest volume in the 150–300 cc range, and prioritizes a fast return to an active lifestyle.
"This is meant for very active women who just want a little bit of extra volume and that beautifully enhanced, natural look," Dr. Earle explains. “All you active moms, dancers, gym-goers and yoga enthusiasts who want to get back to everything quickly, this is for you.”
You may be a strong candidate if you:
- Want subtle enhancement, particularly in the upper pole and cleavage area
- Are active and don’t want a procedure with extended downtime
- Prefer to avoid general anesthesia
- Have firm breast skin without significant drooping

How Is Motiva Preservé Performed?
Motiva Preservé follows a precise three-step process, all performed through a single 2.5 cm incision in the inframammary fold (the natural crease beneath the breast).
Step 1: Channel Creation. Dr. Earle inserts a blunt-tipped channel separator through the incision. Without cutting through the nerves, ligaments, muscle or breast tissue, the channel separator creates a path above the muscle, exactly where the implant needs to go.
Step 2: Pocket Formation. Next, a specialized balloon device is inserted into the channel. Dr. Earle inflates the balloon to match the planned implant volume, gently expanding the tissue to create a precisely sized pocket. Again, nothing sharp, and no cautery or heat. Once the pocket is formed, the balloon is deflated and removed.
Step 3: Implant Placement. A super-soft Motiva Ergonomix implant is placed using a no-touch technique. The implant goes directly from its sterile packaging into the funnel, reducing the risk of capsular contracture, infection and other complications. The Ergonomix implant is used because it’s compressible enough to be inserted through the tiny incision.
From start to finish, Preservé breast augmentation takes approximately 20–30 minutes to complete and is performed under sedation. That means you can return home and start your recovery without the lingering effects of general anesthesia slowing you down.

What Is Recovery Like After Motiva Preservé?
One of the biggest differences between traditional breast augmentation and tissue-preserving techniques like Preservé is the recovery process. Because every structure in the breast is preserved, pain is minimal, and you’ll get back to normal activities faster. Most patients can expect some version of the following timeline after surgery.
Day of Procedure
Most patients are up and moving the same day with little pain or discomfort. In Costa Rica, Dr. Earle saw a Preservé patient have surgery at 10 a.m. and attend a social event later that same evening.
Days 1-7
Mild soreness and swelling may be present. Most patients manage comfortably with over-the-counter medications. No drains, and light daily activities can be resumed immediately.
Week 2
Most patients are cleared to resume all activities (including weightlifting, swimming and yoga) after two weeks.
Weeks 3-6
Healing is mostly complete, but the implants continue to settle. Patients start to notice and appreciate their final breast shape around this time.
Weeks 6-12
Final results are visible. The Ergonomix implant, known for its ultra-soft feel, provides a beautiful, athletic, and naturally enhanced look.
Does Motiva Preservé Interfere
With Breastfeeding or Sensation?
In most cases, no. The goal of Preservé is to preserve your anatomy as much as possible. Because it protects nerve pathways and creates a pocket with minimal disruption of glandular tissue, it has a much lower risk of impacting sensation and the ability to breastfeed.
