Rib remodeling is a cosmetic waist-narrowing procedure that reshapes the lower ribcage to create a more defined waist-to-hip ratio. In this procedure, the “floating” ribs (the 10th, 11th and 12th ribs, which aren’t attached to the sternum) are carefully repositioned to reduce the circumference of the waist. No ribs are removed, so they remain intact and functional.
Rib remodeling may also be described as waist-narrowing surgery, Rib Sculpt, RibXCar, “Wasp Waist,” or “Barbie Waist” surgery. How Is Rib Remodeling Different from Rib Removal?
Rib remodeling does not remove ribs. Rib removal surgery, also called costectomy, does. That's the single most important distinction for anyone researching this procedure, because they are often confused online and the risk profiles are not comparable.

“Rib removal is a lot more invasive,” explains Dr. Earle, “because you’re actually taking ribs out of the body. And it involves more risks: bleeding, pneuomothorax, things like that. The rib remodel procedure is not rib removal. We are simply reshaping the ribs.”

Floating ribs are the last two pairs of ribs at the bottom of the rib cage. Unlike other ribs, they don’t connect to the sternum and have no veins, arteries or nerves attached to them. “They’re the ‘safe’ ribs,” Dr. Earle says. “Floating ribs” technically refers to the 11th and 12th ribs, but most people lack the attachment that connects the 10th rib to the one above it (59% of people, according to one study), making it function as a floating rib.

How Does the Rib Remodel Procedure Work?
Dr. Earle performs rib remodeling under general anesthesia using a Piezotome, an ultrasonic tool that creates precise, controlled fractures without damaging surrounding soft tissue. Six tiny needle-hole access points are made, three per side, one over each of ribs 10, 11 and 12.
Through each access point, the Piezotome creates a shallow fracture on the outer cortex of the rib only. The inner cortex remains intact. That controlled "greenstick" fracture is what allows Dr. Earle to bend the rib inward and re-angle it, bringing the outer circumference of the waist down.
Because ribs 10, 11 and 12 are floating ribs, they have no attachment at the front of the chest and no neurovascular bundle running beneath them, which means no arteries, no veins and no nerves are involved. That anatomy is what makes rib remodeling both low-pain and safer than older-generation procedures that removed actual ribs.

What Is the Rib Remodel Recovery Timeline?
Recovery from rib remodeling is less painful than most patients expect, because the floating ribs have no associated nerves. The real recovery commitment is the corset. Here’s a breakdown of what the recovery timeline looks like after your ribs are reshaped:
- Week 1: Soreness at the access points is mild. Most patients describe it as tightness, not sharp pain. The medical corset is worn 24 hours a day from day one. Walking is encouraged; lifting and twisting are not.
- Weeks 2 to 4: Corset use continues for 23 hours a day. Swelling along the waistline begins to settle. Most non-physical desk work is possible after two weeks.
- Weeks 4 to 8: Light cardio can resume based on Dr. Earle's clearance at the six-week visit. The corset stays on. The waist begins to visibly hold its new shape.
- Weeks 8 to 12: Strength training can gradually be reintroduced to your routine. The ribs are in the final phase of healing into their new position.
- Three months: The corset can come off after three months. Final waist shape becomes visible. The needle-hole access points are healed and virtually invisible.
"If you're going to do this procedure, you have to commit to wearing a corset for three months,” Dr. Earle says. “This is non-negotiable. If you cannot commit to that, don't do the procedure, because it's not going to work."

What Results Can I Expect After Rib Remodeling?
Rib remodeling produces nearly instantaneous results. You will notice a narrower and more defined waistline immediately after surgery, with a coveted hourglass shape that many patients seek. Is Rib Remodeling Safe?
Rib remodeling is safe when three conditions are met: only the floating ribs (10, 11, 12) are addressed, the work is done with precise ultrasonic technology like the Piezotome, and the patient is under general anesthesia to eliminate movement during the controlled fracture.
At PURE, all three conditions are standard. Dr. Earle does not perform rib remodeling on the upper ribs, does not use improvised tools and does not perform the procedure under sedation. Realistic risks, as with any surgical procedure, include bruising, temporary discomfort and asymmetry if the corset isn't worn consistently. Serious complications like pneumothorax (collapsed lung) and major bleeding are extremely rare with the rib remodel procedure.

Can Rib Remodeling Be Performed With Other Procedures?
Yes, and combining rib remodeling with a BBL and Lipo 360 is what Dr. Earle calls BBL 2.0, and it's how most PURE patients have the procedure done.
This combination allows Dr. Earle to address the waistline from multiple angles. Liposuction removes excess fat from the entire midsection, HD etching improves definition and rib remodeling brings in the waist beautifully.
