That doesn’t mean every patient wants the same look. Some want the smallest possible change with the fastest possible recovery, while others do want more noticeable volume and are happy to trade a few weeks of downtime to get it.

What’s the Difference Between Preservé & Traditional Breast Augmentation?
Traditional breast augmentation follows a familiar sequence: You get put under general anesthesia, an incision is made (usually in the breast crease), and your surgeon carefully dissects the tissue to create space for the implant. Implants are then inserted either above or below the muscle before the incisions are closed. The entire process takes about one to two hours, and you wake up with a new look, ready to go home and start the recovery process.
While placement options, incision preferences and implant technology have all evolved with long-term safety data, the basic incision-dissection-placement approach has remained the same. Motiva’s low-impact, tissue-sparing technique with Preservé has changed the experience by rethinking how implants are introduced, reducing surgical disruption at every step.
It’s breast augmentation for women who want the change, but not the ordeal.
Unlike traditional breast augmentation, Preservé uses specialized instruments and an inflatable balloon to gently push the tissues aside, creating space without cutting through muscle, ligaments or nerves. “We’re able to preserve all of the natural ligaments that go around the breast,” explains Dr. Earle. “We’re not even using heat or cautery to create the pocket. And we’re all preserving the nerves, muscle, everything. That’s why it’s called Preservé.”
The tools and techniques used during Preservé breast augmentation mean the incisions are smaller, the recovery is quicker, and it can be performed using local anesthesia. “You're not going to have to deal with all the stuff that comes along with general anesthesia,” says Dr. Earle. “No need for labs and EKGs and all that other stuff. It’s very quick and very easy to prepare for.”
Before You Compare Procedures, Compare Your Goals
Before you start thinking about Preservé versus traditional breast augmentation, it helps to define what you actually want to achieve.
What kind of change are you hoping to make? Bigger is only one option. Some patients want restored volume after pregnancy or weight loss. Some want more symmetry between breasts that were never quite even. Some want a lift more than they want size. Naming your desired outcome narrows the field, because not every goal is a job for breast implants.
How much change do you actually want? Traditional augmentation typically adds 300 to 500 cc of volume per breast. Preservé tops out around 300 cc, with most patients landing near 200 cc. "This is meant for that very active woman who just wants a little bit of that extra volume, especially in that cleavage area," Dr. Earle explains. "It's not meant for a super drastic look."
How much downtime can you manage? Traditional breast augmentation performed by an experienced surgeon is safe and produces beautiful results. But it does involve more: general anesthesia, a larger incision, and, frequently, work beneath the muscle. Preservé is built around preserving anatomy, which is why it can be done under sedation with a two-week recovery.
What does your anatomy actually allow? Factors like your existing breast tissue, skin laxity, chest width and nipple position matter more than the name of the procedure. Preservé requires a relatively firm breast envelope with minimal sagging. A patient with significant skin laxity or a low nipple position may get a disappointing result from any augmentation done alone, no matter how advanced the technique.

When Traditional Breast Augmentation Is the Better Choice
Traditional augmentation is the right call when your goals or anatomy fall outside what a tissue-preserving approach can deliver.
- You Want a Larger Result: If you're hoping to go from an A cup to a D cup, Preservé won’t be able to get you there. The implant size needed exceeds what the technique supports. Traditional augmentation allows you to achieve a larger, more dramatic result.
- You Already Have Larger Breasts: Patients with naturally larger breasts are often better served by a traditional approach, especially if the breast tissue is heavy, the skin is stretched, or the nipple points downward. In these cases, the goal is usually improved lift or support rather than a simple volume increase.
- You’re Replacing Existing Implants: Preservé is designed for first-time, or primary, breast augmentation. If you've had implants before, your internal anatomy has already been changed, which removes the tissue-preserving benefit the technique is built on.
- You Need a Breast Lift: Preservé can add subtle volume, but it cannot reposition the nipple or lift sagging tissues. If your breasts sit lower on the chest, traditional breast augmentation with a lift is more likely to create a satisfying result.
When Preservé Is the Right Choice
Preservé is not meant for every breast augmentation patient, but for the right anatomy and goals, it can make the experience feel smoother and less disruptive.
- You Want Subtle Enhancement: Using the FDA-approved Motiva Ergonomix implant and above-the-muscle placement, Preservé is designed to provide modest upper-pole fullness, a little more cleavage and a natural-looking result.
- You Have Minimal Sagging: Ideal patients should have minimal sagging (grade zero to one ptosis) and a relatively tight breast envelope to support the implant. When the surrounding tissue is firm, results are more controlled and predictable.
- You Want a Faster Return to Normal Activities: Preservé provides a gentler approach to breast augmentation with a quick, two-week recovery, so active moms, ballet dancers, pilates enthusiasts and gym-goers can resume exercise and heavy lifting sooner.
- You’d Rather Avoid General Anesthesia: General anesthesia is considered safe when performed by a board-certified anesthesiologist in a JCAHO-accredited facility, but it still carries potential risks and side effects. Preservé can be performed using local anesthesia and avoids many of these risks. It also doesn’t require labs, EKGs or intubation, all of which come with a traditional setup under general anesthesia.
Will Either Procedure Affect Breastfeeding or Sensation?
Both procedures can be performed in ways that protect nerve pathways and glandular tissue, so the risk of sensation loss or an inability to breastfeed is generally low. Preservé is built specifically around preserving these structures, since it avoids cutting through nerves and creates the pocket with minimal disruption to the surrounding glands. Traditional augmentation through an inframammary incision also has a strong track record here.
